The United States passport is issued to citizens and nationals of the United States of America, exclusively by the US Department of State. It gets considered as the most powerful passport in the world and recognized by most countries across the globe. With a US passport, you can travel to almost 155 countries without a visa, and the numbers continue to increase. If you are an American citizen, all you have to do is fill Form DS-11 to get your passport issued. However, it is necessary to do that correctly so that you avoid any delays and rejection. What is a DS-11 form?

DS-11 Form is also known as the US Passport Application Form and is one of the most widely used application forms in the United States. It gets provided to US citizens by the United States Department of State.

Who Needs Form DS-11?

When to re-file DS-11?

To begin with, if a passport gets issued to a child, it is valid only for five years as opposed to 10 years for adults. Also, if the child is under 16 years of age before the passport expires, using form DS 82 for renewal purposes is not an option. Therefore, if your first US passport gets issued before turning 10, you cannot technically file for renewal. In such a case, you have to use the passport application form DS 11 to file for a new passport.

Similarly, if your passport gets lost, stolen or damaged, you cannot apply for renewal and have to again opt for the DS 11 US passport application route to get a new passport. The various conditions in which you have to DS 11 US passport application are as follows -

  • You’re applying for a US passport for the very first time.
  • You are under the age of 16 at the time of applying.
  • You were under the age of 16 when you first received your first US passport.
  • Your latest US passport got issued no later than 15 years from the current date, and it got expired.
  • You have lost, severely damaged your recent passport, or it got stolen.
  • You changed your name since the last time your passport got issued, but you are unable to document the same.
  • You are requesting a gender marker change.

If you do not fall into any of the above categories and are applying for renewal, then you should go for DS 82 instead.

DS 11 passport document requirement -

The DS 11 passport documents that get required while submitting the form are as follows -

Documents that prove you are a US citizen -

To get a US passport, you have to be an American citizen, either by birth or through naturalization. Several documents can prove your American citizenship status but, that varies depending on certain factors like whether you were born in the US, born outside of the US to American parents, or became a US citizen through naturalization. Some of the documents you can submit to prove your citizenship includes the US hospital birth record, certificate of naturalization, certificate of citizenship, or consular report of birth abroad/certificate of birth.

Secondary documents of citizenship - You may also have to submit your parent’s naturalization certificate, FS-240, foreign birth certificate, Letter of No Record or any other similar document that is applicable in your case.

ID documents -

To prove your identity, you can submit any US government-issued card that has your name, photo, and date of birth printed on it. But you have to ensure that the ID proof you are submitting are permanent ones and not temporary.

  • State-issued driver’s license
  • Permanent resident card
  • Certificate of naturalization
  • Certificate of citizenship
  • Government employee ID card

You will also have to submit two recent photographs that meet the requirements, such as, the photo should not have been taken before six months from the submission of the form. It should be 2X2 inches in size, identical to each other and provides a full-face front view with a plain white background.

How to fill out passport application?

Before we answer how to fill out a passport application, you should know that DS 11 is a 6-page form out of which four pages cover instructions. So all you have to fill is two pages. Moreover, you have to fill the DS 11 passport form online and submit it on the portal. Then you can print it out and visit the US passport office or authorized agent (US Embassy, if you apply from overseas). Here we are listing the steps required to complete the DS 11 passport form online -

First page -

On the first page, you have to choose among the various options which include whether you are applying for a passport card, a passport book, or for both. Once you select the applicable option, you have to fill out nine fields that are as follows -

  • Name (first, middle, and last)
  • Date of Birth
  • Gender
  • Place of Birth
  • Social security number
  • Email address
  • Primary contact phone number
  • Mailing address
  • List of all other names you have used legally

After you complete this section, you will have to provide some additional information related to the identification document that you will be submitting with the DS 11 online form.

Second Page -

The second page includes 11 more questions about you and your details. Even providing intricate details like hair color for passport application can play an important role in the successful completion of the form.

After filling the form online, you have to visit a US State Travel Departments qualified centre and submit it in-person.

If you are applying from abroad, you need to visit the US Embassy in that country.
